Need Help from Polk

I bought another Polk Momo 124 and the roadshop installed it for me.. Unfortunately they end up throwing away the box which includes the UPC. I need the UPC to send it for the $50 dollar rebate. I got the receipt from the store.. the form but I dont have the UPC. Is there a way I can just send my Receipt, Form and my serial number? $50 dollars is $50 dollars... I wanted to get another 124 and that would greatly help.

rebate? -- polk's got a 50 buck rebate on the momo's ?? does this apply to NIB momo's puchased anywhere or just from authorized dealers? and is it 50 bucks off a 2nd momo or off each or what?

sorry to bump in on your post but i couldn't turn down asking about a rebate.

I think its from authorized dealers.. it ask for the receipt plus a form printed by the dealer and yes its 50 bucks per polk momo mm12. The one with the enclosed sub box. I got two and MAN they make me so happy.. I don't miss my 2 DB's as much anymore
