hi i work at cicuit city and one of my custemers has lost his lost your warr. form how and who do i need to send him to?, if you could give me a number or a adress to mail his sub to, or somthing i would be very greatful. thanks

Not a problem. As long as the customer has his sales receipt
then this is all he needs. If he needs warranty service, return the speakers to Polk Audio 2550 Britannia Boulevard Suite D San Diego, CA. 92154. Include a short note stating the problem with the speakers, return shipping address, daytime phone number along with a copy of his receipt.
