Looking at the pic in your first post, that's exactly what they did. I see the grommet just in front of the zip tie.

The 6501 component set comes with leads for tweeters and mids, to run to the outboard crossovers.
What settings? Does your head unit have hp filters, or are you using the amps'? What are the speakers crossed over at, if at all?
You won't want to use the preEq feature on the C400.4- that was for running a set of MM6 components all active. If you use a C400.4 to run front and rear speakers you'll only hear the tweets up front...

The Pre- EQ switch was meant for the older MM6 (Momo classic) component set. Is that what you have? If not, I wouldn't use the "Polk EQ" on the amp.
Does the amp have an adjustable high pass crossover? If not, is there a fixed hp frequency?

Have you tried just running the front speakers without the sub? If the sub is bad it could be shorting the back channels of the amp and it'll go into protect mode. Or possibly a short in the speaker...
